Purpose:
This form is used to verify student eligibility for either institutional aid or endowment scholarships. The awarding of the scholarship is based on need, availability of funds, and guidelines listed on the Institutional Aid page. The student is responsible for reading and following scholarship guidelines.

• Students cannot be awarded more than one of each type of scholarship (Institutional Aid and Endowment), but may receive one if eligible.
